To help prevent the spread of COVID-19, you must complete Wexford Hockey Club Health Questionnaire Form. Certain responses will preclude you from returning to training/playing/coaching with Wexford Hockey Club in order to protect the health and safety of other members. A form must be completed for each individual member.
