The Knights of Columbus in Eldred have generously offered a supply of winter coats for children in need.  If your child(ren) would like to receive a coat, please complete the following information.  If you have multiple children, please complete a separate form for each child.  Please note, all information is kept confidential and is not released to the Knights of Columbus or any other organization, but is used solely by the District to disperse the coats.
