Participant's Personal Liability Release and Photo, Video, and Image Use Release Agreement

In consideration of being allowed to participate in workshops, classes, and tournaments hosted by Fisherman's Haven Kids Association Fishing University, I acknowledge and agree that the risk of injury from these events is significant, including the potential for permanent paralysis and death. While particular rules, equipment, and personal discipline may reduce this risk, the risk of injury does exist. I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of the release or others, and I assume full responsibility for my participation. I willingly agree to comply with the stated terms and conditions for participation in these events.

I, for myself and on behalf of my heirs, assigns, personal representative, and next of kin, hereby release, indemnify, and hold harmless Fisherman's Haven Kids Association Fishing University, its officers, agents, and/or employees, sponsoring agents, sponsors, advertisers, and, if applicable, the owners or lessors of premises used to conduct these events (Releases) with respect to any and all injury, disability, death, or loss or damage to person or property, whether arising from the negligence of the release or otherwise, to the fullest extent permitted by law.

I also acknowledge and agree that Fisherman's Haven Kids Association Fishing University, its representatives, or anyone authorized by them may take photographs or make audio or video recordings of the workshops, classes, and tournaments. I hereby grant to Fisherman's Haven Kids Association Fishing University the right to use, reproduce, distribute, and display these photographs, audio recordings, or video recordings of me for educational and promotional purposes, including, but not limited to, print materials, online publications, social media, and presentations.

I understand that parents or guardians may assist their children in these events and tournaments, with the exception of the actual reeling in of the fish, which should be done solely by the registered participant.

Participant's Agreement:

By signing up for the workshops, classes, and tournaments hosted by Fisherman's Haven Kids Association Fishing University, I confirm that I have read and understood the terms of the Participant's Personal Liability Release and Photo, Video, and Image Use Release Agreement provided above. I agree to abide by the terms and conditions outlined in this agreement.
