The undersigned acknowledges receipt of equipment from the San Joaquin Delta College Digital Media department inventory, and by signature, certifies that it is in good working order and condition.

Further, you agree to be fully responsible for this equipment and to render proper care of it.

The borrower agrees to make provisions for the protection of the equipment from the elements, and to be financially responsible to San Joaquin Delta College for any equipment that is lost, stolen, damaged or rendered inoperative due to the impact damage, abuse, negligence, accident, or misuse.

It is understood that San Joaquin Delta College, Digital Media department, Art, Humanities, and Multimedia department retain title to the listed herein.

The equipment is to be returned in good working order and condition to the same location from which it was borrowed, by the listed due date.

Lost accessories must be replaced.

All equipment requests are to be made after the 4th week of the current semester (no checkouts until after drop date). Unless given special approval to do so. It is understood that if equipment is misused, or unused and no assignments are being submitted after equipment checkout that privilege's will be lost and deemed as equipment abuse.
