This form is to collect a list of families who would like a space in the after school program for the 2022-2023 school year.

Program Goal
The goal of the after school program is to encourage the love of learning, expose students to real experiences, develop social skills and build competence in academic abilities.  In order to achieve our goal, we need the full cooperation and support of every parent and child. The program serves students in grades TK-8.
 
Parent Responsibility:
Direct your child to come directly to the program everyday. Your child must be signed in and out by an authorized adult. Call and provide written notice when your child is absent. Discuss the rules with your child. Respect program staff. Pick your child up on time.
 
Participation Requirement
Students are required to attend the program every day for maximum benefit.  For continued funding, our program is contingent on all students attending everyday.  Students receive academic support, enrichment, and physical activity in the after school program.  Students are expected to complete each class rotation daily.  If absences or early pick up becomes a pattern, students will be asked to leave the program so students on the waiting list may have an opportunity for full participation.
 
Tardiness and Pick Up
The after school program starts at the end of the school day and closes promptly at 6:00 P.M, You may be charged a late fee for late pick-up. Repeated late pick-ups may result in cancellation of your child’s enrollment in the program.  Excessive tardiness is considered to be more than 3 times in one school year.  
 
Be sure to click "SUBMIT" when finished with the interest form. Complete this application as soon as possible and be sure to click "SUBMIT" when you are finished.  Enrollment is based on the following priorities: 1. Homeless and Foster Youth 2. Each school's academic and social needs 3. Siblings and 4. First come, first serve basis. Programs will enroll until the program reaches capacity.  A waiting list will be developed if capacity is reached. The waiting list is maintained for each grade level/rotation group throughout the academic school year.  
 
This interest form is valid for any school your child may attend for the 2022-2023 school year in the Oxnard School District.

Completion of this application does not guarantee acceptance into the After School Program.  You will be notified before the start of school if your application is accepted.
