TRAINING RISKS: I understand the risks involved when participating in SparkHub's athletic activities. I agree that the participant(s), whether myself, a child, or a minor, are medically and physically able to be involved in such an athletic activities. I understand that I should consult a physician before any type of athletic activities. I discharge, waive, release any claims for losses or damages, mental and/or physical injury, death, that may take place during or after participation with SparkHub. I agree to assume these risks. I agree to indemnify and hold harmless SparkHub or any employee, volunteer, coach associated with SparkHub from any losses, damages, costs, injuries, death that may occur with SparkHub activity in any facility or location (outdoor or indoor) in which SparkHub uses. I agree that I am responsible and liable for such costs, losses, liabilities, damages, injuries, deaths that may occur and I agree not to sue SparkHub or employee, volunteer, coach, or person associated with SparkHub.

TUITION: We are a non-profit organization. There is no required tuition for this class.

CANCELLATION POLICY: Please notify SparkHub 7 days in advance if you are discontinuing the program. If you discontinue the program, you will have to re-apply if you plan to re-join in the future. Return is not guaranteed and will depend on availability and current wait-list.

ATTENDANCE POLICY: Please notify SparkHub 1 day in advance if you’re missing a class.

MAKE UP POLICY: We have a NO MAKE UP POLICY. 

HOLIDAY/TRAINING SCHEDULE: Our Holiday schedule will be notified through email. SparkHub holds the right to cancel any classes without refund due to personal reasons, such as illness, from the volunteers or coaches.

PHOTO RELEASE WAIVER: I hereby grant SparkHub the permission to use my likeness in a photograph, video, or other digital media (“photo”) in any and all of its publications, including web-based publications, without payment or other consideration. I understand and agree that all photos will become the property of SparkHub and will not be returned. I hereby irrevocably authorize SparkHub to edit, alter, copy, exhibit, publish, or distribute these photos for any lawful purpose. In addition, I waive any right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photo. I hereby hold harmless, release, and forever discharge the SparkHub from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.
