Use this form to register for the MSVMA Chamber Festival. Please fill out the form completely for EACH ensemble performing at the Festival. Be sure to click on the link to the invoice at the end of the form and MAIL completed invoice (with signatures) and payment to the MSVMA Office at P.O. Box 1131, Big Rapids, MI 49307. You may have multiple invoices if you have more than one ensemble performing, but combining them into one payment is fine. The fee for each performing ensemble with 4-12 is $15 and 13-24 is $25.
