Onside Athletics is determined to provide a safe and fun camp experience for all campers. In light of Covid-19 and the state of our communities, we are requiring this form is filled out on behalf of each camper within 24 hours prior to their attendance at camp.  Please answer all questions as honestly as possible.  If you answer "yes" to any of the following questions, you should consider keeping your child home.  If we determine that there is cause for concern based on your responses, we will ask that your child does not attend camp in order to preserve the safety of staff and participants.
