Trinity Lutheran Church is following guidance from the U.S. Centers for Disease Control and Prevention, the Louisiana State Department of Health and other agencies, and has implemented social distancing measures, increased hand-washing and other hygienic practices, and frequent cleaning of common surfaces. Despite such precautions, the risks associated with transmission of COVID-19 remain. The nature of many Sunday school activities makes it difficult to observe all of the recommended precautions all of the time, but we at Trinity will do our best to encourage frequent hand washing/sanitizing, wearing of masks, social-distancing, temperature checks, and frequent cleaning of common surfaces. As a result of participation in the activities of the ministry and proximity to other participants, you or your child may be exposed to the risk of infection of communicable diseases, including COVID-19. By signing this form and allowing your child to participate in Sunday school, you knowingly and voluntarily assume the risk associated with participation. You also knowingly and voluntarily agree that your child will be able to fully participate in all Sunday school activities and receive all hand-outs including lesson materials, crafts, and individually-packaged snacks.
