1. Consent Form:

Email the form to Transcript Assistant, Marlyn Adames, at (madames@fcps.edu).

2. Payment:

Submit payment of $5.00 for each transcript requested trough this link

You do NOT need to have an active MySchoolBucks account to pay.

Increase the transcript quantity to reflect the total amount if requesting more than one.

Once paid, please continue to step 3.

3. Transcript Request Form:

Continue to complete this form and submit it.

● You must allow at least 30 days for processing and mailing transcripts. We cannot guarantee transcript delivery by your deadlines. We do not utilize express mail system and transcripts cannot be sent via email, Common App, Naviance, eDocs and/or Parchment.
If you are close to the deadline, it is strongly recommended to pick up the transcript from our school and send it via express mail in your local post office.
