I am a parent or legal guardian of this all-star candidate who is currently participating in Ahwatukee Little League. I understand I must submit this application in order for this candidate to be considered for all-stars.

This candidate has or is expected to participate in at least 60% of A.L.L. regular season games that do not directly conflict with school baseball games or practices.

I will abide by all of the policies and procedures described in the all-star procedures memo currently on the league's website (https://bsbproduction.s3.amazonaws.com/portals/28948/docs/2019%20all%20star%20selection%20process.pdf).

If selected, the candidate will fully participate in the all-star program. He/She will be available for all practices and games from June 7 (subject to change) through the end of competition (as explained in memo) except for foreseeable instances on the dates I have entered below. I understand that all-star teams practice 5 to 7 days per week and that absences, whether excused or unexcused, could affect playing time.

I understand that each selected player will be required to pay a $250 participation fee. Scholarships are possible in cases of demonstrable financial hardship.

I have discussed with this candidate the commitment involved in all-stars.

The candidate and I understand that this application is only a request to be considered for participation and in no way a guarantee of selection.
