By completing and summiting this RSVP, you are agreeing to the following terms and conditions in our waiver agreement:

As the legal parent or guardian of the youth performer named above, I grant permission for my child to participate in Summit Dance Challenge's Nationals Opening Performance.

I understand that participating carries inherent risks of injury, and I voluntarily assume all risks and responsibilities associated with my child's involvement in the performance. I acknowledge that Summit Dance Challenge, its organizers, instructors, and representatives, hereafter referred to as the "Event Organizers," are not responsible for any injury or harm that may occur during the preparation or performance.

I agree to hold Summit Dance Challenge and its representatives harmless from any liability, claims, demands, costs, or judgments related to my child's participation in the performance.

I further understand and agree that my child may be removed from the performance if they fail to learn the choreography, do not adhere to the performance dress code, are unable to participate in rehearsals, or exhibit unsportsmanlike behavior, as determined by the Organizers.

I acknowledge that I have read, understand, and agree to the terms and conditions outlined in this Youth Performer Waiver Agreement. My child's participation in the performance is entirely voluntary, and I consent to them participating with full knowledge of the risks involved.
