1. Working title of the submitted project *
2. Name of the contestant (Name of organisation or personal name) *
Please provide the name of the applicant or the name of the individual.
3. Contact information *
Surname and first name of the contact person (or name in case of organisation / business entity), telephone number, e-mail.
4. Website / Instagram (if available).
5. Purpose (s) of project implementation. *
What goal (s) do you want to achieve when implementing the project, and to what extent will it correspond to the goal of our competition (clause II.1 of the Regulations). The goal can be, for example, anticipated social change,  sharing educational or cultural content, and so on. Write at least 1 goal. (max. 1000 characters).
6. Description of planned activities. *
Describe briefly and concisely the items that will make up your project and how they will be implemented. Write whether the events will be online or offline. (Max. Approx. 1500 characters).
7. Recipients of the project. *
Describe the target group / groups of recipients to whom the project is aimed. Write how you plan to reach your audience. Consider whether the activities will be registered / have a recruitment list, and if so, how. (Max. 1000 characters)
8. Place of realisation of project activity. *
Write where / from where you plan to operate. You can mark more than 1 answer.

9. Project result (s). *
Briefly write what will be the result of the planned activities. The results identify the “goods and services” that will be created as a result of the project and will be delivered to the target group, for example, “there will be an exhibition / seminar at XYZ for ZXY recipients”. (max. about 800 characters)
10. Date of planned events - schedule. *
Make the short list of planned activities (dates may be approximate).
11. Project estimate. *
Write down what costs will be associated with the project, such as staff (salaries), materials (if necessary) and others, if necessary. Please note that you can (but do not have to) use our technical and promotional support (including graphics) in addition to grant costs. The amount of the grant is PLN 2,000 brutto, we will not pay the participants' expenses, the cost estimate is only general and informative.
12. Information about the author of the project (optional).
Here you can enter additional information about yourself / your business, previous projects, provide additional links to websites, etc. (max. 1000 characters)
13. Additional materials (optional).
Here you can optionally include links to additional visual information that will be useful for processing your application. If you want, you can attach additional materials, such as photos, presentations, videos, links, etc. Please attach them, for example, to a Google drive or send them, for example via we transfer, by placing a link here.
