2023 Manly Life Youth & H3O Permission Form 

Parental Permission Forms are required for all participating children who attend Manly Life & H30 Youth Group on Fridays. This form allows parents to be familiar with our policies and procedures, provide important medical and dietary information, and also provide consent for various areas, like photo/video at Youth Group. There is space for your young person’s information such as birthdays and favourite snack to allow for pastoral involvement from the Youth Pastor over the year.

Program Details:

Time: 6pm-7:45pm Ages: Years: Junior (Yr 4-6) Senior (Yr 7-12)

Every Friday we have free time, a fun activity, food, a Bible talk, and time in small groups. We also have special events and camps throughout the year, plus a youth program on Sundays!

All information is held with strict confidentiality and stored in a secure, private location according to our Privacy Policy.  

You may complete one form per family, so long as each child’s details are all filled out.

Safety and Child Protection Policy

At Manly Life Church, Safety and Child Protection are of the upmost priority and importance. We have a range of policies & procedures keeping our programs safe!

Manly Life complies with all legal requirements and goes above and beyond to ensure Best Practice policies are practiced for the safety and wellbeing of every child. 

Every leader involved in Manly Life Youth Group has gone through a safety screening & application process, a verified, valid Working with Children Check and has completed Safe Ministry Training.

Photo and Video Permission 
I (you, the parent or guardian) give permission for my child/ren to have photos and video taken of them during Manly Life Youth Group program. I understand that these photos will be used only for internal Church media including but not limited to the church website, Facebook/Instagram page, posters, flyers, brochures, noticeboards and announcements.

SIGNATURE OF PERMISSION
I authorise the leaders in charge of Manly Life & H3O Youth Group to arrange for my child to receive such first aid, medical or surgical treatment as deemed necessary at any time during the activities of Manly Life Kids Club. I further authorise the use of Ambulance and/or anaesthetic by a qualified medical practitioner if in his/her judgment it is necessary. I accept responsibility for payment of all expenses associated with such treatment. I agree to indemnify and hold harmless the Baptist Churches of NSW and Manly Life Church against all claims, demands, suits and liability of whatever nature and howsoever arising out of the injury to the child, and the relevant activity being undertaken.
