Kindly complete the registration process in two (2) steps:

STEP 1: Complete and submit this online registration form. Please leave STUDENT'S E-MAIL field blank as an official College
              e-mail address would be created and assigned to the student.

STEP 2: Submit copies of the following documents in a clearly labelled envelope bearing the student's full name, by
              Friday 10th September, 2021 at 4:00 PM, at the College. Please walk with originals to verify the copies:

• Immunization Card
• Computerized Birth Certificate [students with dual citizenship status are required to provide evidence of T&T citizenship]
• SEA Examination Slip
• Baptism Certificate (if applicable)
• Completed Bio-Data Form (available for download on the College website) with passport size photo attached
