Please fill out this form and submit immediately after you have sent your deposit or payment. This will help us in retreat planning. We need to receive your answers to all but the last three questions right away. When you have answers to the last three questions you can edit and resubmit this form.

· Consent Forms (required for every adult and student attending)
· Group registration form submitted by the group leader.
· Every adult attending as a leader or chaperone must turn in a confidential background verification form.
· Every church bringing a group is required to obtain and submit to connect@abcoflash.org a certificate of
insurance from their insurance company naming ABCOFLASH as additionally insured

The Worship Team of WYR is looking for individuals and groups to participate in the sessions. Please indicate what your church might contribute in the sessions. Someone from the planning team will be in touch with you to discuss the possibility of participating. They need to know no later than February 4th.

A Testimony About Handling "Pressure"

During the retreat, we are going to offer "Real Talk" Conversations. The planning team would like you to have a discussion with your group about the pressures they deal with within their lives. (at home, school, family, spiritual, church, etc.) Please list the top three to five pressures your students face.

For the "Real Talk" Conversation we need mature, experienced leaders who are open to facilitating a discussion with a group of 10 - 20 youth. Please give the names (including yourself), phone numbers and email addresses of leaders in your group who have the ability and willingness to possibly lead one of these groups.

A copy of your responses will be emailed to the address you provided.
