Thank you for recognizing a Carmel Clay Schools staff member for their positive contribution to our community.  Honorees will receive a personalized certificate with a message from you/your child about why you are honoring them.  Additionally, principals/supervisors will be notified of their honor and their name will be listed on the Carmel Education Foundation website.  

Your accompanying donation will benefit Carmel Clay students through grants, district-wide programs and scholarships.  We are grateful for your support!

This form will allow you to recognize up to five Honorees.  If you wish to recognize more, please complete an additional form.  Thank you very much!

A printed certificate will be sent to all nominees by December 17, 2021 for nominations received by December 12, 2021.
