PARENTAL CONSENT FOR AN EDUCATIONAL VISIT

CHALFONT ST GILES JUNIOR SCHOOL – YEAR 3

Year 3 Skipping Festival at Amersham School,  Stanley Hill, Amersham, HP7 9HH 

All of the children in Year 3 will be participating in a Skipping Festival at Amersham School on Wednesday 28th February between 8:45am - 11:15am (event time is from 9.15am to 10:45am). The children will be travelling to and from the festival by coach.

The school gates will be open slightly earlier (8:40am) for Year Three on Wednesday 28th February to ensure everyone is on the coach by 9am to get to the festival on time. 

Children will be back for lunchtime so will have a normal lunch. 

Children should come into school wearing their school PE kit (green T-shirt, black shorts or black jogging bottoms and green hoodie) and trainers. 

Children will also need a named water bottle to take with them.

By completing this form I am giving consent for my child to attend the Year 3 Skipping Festival at Amersham School on Wednesday 28th February. I confirm that I have read the relevant information above. Consent forms to be completed and returned by Thursday 8th February. 

I agree to his / her participation in the activities described and I acknowledge the need for and expect him / her to behave responsibly.  I can confirm that the school has current contact and medical details for my child.
