If you are moving out of the Ames Community School District please take a moment to fill out this form. This information will help ensure that your students' records are sent to the correct school once that school sends a records request. By filling out this form, your students will not be added to a class list and will be removed from Infinite Campus once a records request is received. 
Enrollment will be ended in the Ames Schools once a records request is received from the new school.
