You should use this form to book Breakfast and After School Club sessions at Tree Tops for the second half of the summer term, from Monday 7th June 2021 to Friday 23rd July 2021 inclusive.

On Friday 23rd July, school will close early at 1:30 pm. Tree Tops will be running only one After School Club session from 1:30 - 4:30 pm. The charge for this session is £15.

Please complete a separate form for each child you would like a book for.

Section 2 asks for information about your child, including parental and emergency contact telephone numbers and medical and dietary details. It also asks for information about who is authorised to collect your child from a Tree Tops session.

Section 3 is the Tree Tops Home School Agreement. The Home School Agreement includes important information regarding the booking and payment process for Tree Tops, the school's commitment and the terms and conditions of your booking. The Agreement is available on the school website via the link provided. Please read it carefully and sign and date the declaration.

Section 4 is the booking form where you can select the Tree Tops sessions you would like your child to attend. To adhere to government and Local Authority guidelines, the children attending Tree Tops will be placed in group bubbles of no more than 15. In order to maintain consistency within these groups and to minimise the risk of transmission of COVID-19, we must ask you to commit to the same sessions every week from Monday 7th June 2021 to Friday 23rd July 2021 inclusive. Unfortunately, it will not be possible to change the sessions your child attends from week to week.
