Please fill out this form anytime you have an update to the number of students participating in Feeding Our Kids at your school.

If an increase is requested of more than 5 bags or 20% of your previous number (whichever is greater), the FOK Board of Directors needs to approve the request. Also, if more than 2 increases are requested per quarter, the FOK Board of Directors will need to review additional requests.

If the total number of students across all schools is already at the budgeted amount, increase requests cannot be accepted until (1) the Board of Directors approves an increase in the overall number or (2) other schools decrease their numbers.
