Fill out this form in its entirety and submit it for the funding request. Any blanks left on this form will be assumed to represent null values. We review all funding requests twice a quarter (with the exception of fall quarter, which is only once) and respond within that frame on whether or not we have funded an organization.

Funding Requests for this round will be due November 9th at 11:59PM. The form will be closed between the due date and the funding review meeting. We will be reviewing all submitted funding requests on November 11th beginning at 7:00PM.
