WVDDC 5 Year State Plan Survey

The WV Developmental Disabilities Council wants to hear from people with developmental disabilities, their families, and other important people in West Virginia as they start planning for the next 5-Year State Plan. The Council is gathering information from different places in the state, and this survey is one way to share your thoughts.
