If registering multiple players, please submit a separate form for each player. Once form is filled out, please etransfer full payment to cpcanadians.cchl@gmail.com and provide player name and camp selected. Camp spots are limited and non-refundable.

The applicant will not hold the owners and staff of the Carleton Place Canadians Hockey Club liable/responsible for damages, Ioss or injury which may occur and hereby release the Carleton Place Canadians Hockey Club from any actions which may occur while attending the Future Canadians Summer Day Camp. This release covers travel to and from the arena as well as any and activities on and off the ice
