PREP Registration and Parental Permission (May 13th-June 10th 2024 online session)
Please have a parent or guardian fill out this form.

The Basement Teen Center is excited to offer online PREP [Personal Responsibility Education Program], an evidence-based curriculum that builds skills to prevent STI and HIV transmission and unintended pregnancy. Information alone won’t prevent teens from engaging in high-risk behaviors, but evidence shows that having accurate information about the consequences of unprotected sex helps teens make better choices, especially when parents/guardians reinforce the program’s information with clearly communicated expectations.

This program is designed to engage teens in conversations that encourage them to take charge of their physical, emotional, and sexual health. Anyone who successfully completes the course (at least 6 out of the 8 classes) will get a $125 stipend! All teens 12-17 are welcome to participate, regardless of any attendance at the Basement Teen Center.

PREP will begin Monday May 13th, and continue on Mondays and Wednesdays, May 15th, 20th, 22nd, (no PREP on May 27th), 29th, June 3rd, 5th, and 10th. Classes will be online, from 6-7 p.m.

This 'PREP Registration and Parental Permission' form is required. Please register by May 10th.

Thank you for your help, and for encouraging your youth to take charge of their physical, emotional, and sexual health.

Sincerely,
Robert Morgan
