AMA Birthday Party Application Form
Please complete this form to book AMA Taekwondo Birthday Party with Non-Refundable DEPOSIT of $50.00 upon signing/completing the application form in order to guarantee the reservation for the spot.

FEE SCHEDULE
  • $225.00 / NON-MEMBERS up to 15 students $15/additional per child
  • $150.00 / MEMBERS up to 15 students $10/additional per child
Martial Arts Birthday Party Guidelines
  • Participants must be 4y.o. and older up to 25 guests maximum. 
  • You provide all food, drinks, paper goods and utensils. Your own Decorations are welcome. May come not earlier than 30mins. before party.
  • We require all the participants have a Signed PERMISSION SLIP before participation. 
  • Non-Refundable DEPOSIT of $50.00 upon signing the application form in order to guarantee the reservation for the spot.

90 mins. Total Martial Arts Birthday Party includes:
  • Birthday Party Set-up w/ in-house decoration and clean-up included. 
  • Every guests will receive a VIP Pass (1 week free Taekwondo class for non-members); nice addition to your party favor.
  • Boards for Celebrant and guests to break in class (if preferred)
  • Samurai sword cake cutting (if preferred)  
Martial Arts Birthday Party Schedule:
  • Birthday celebrant welcoming guests
  • Mini Taekwondo instruction w/ games
  • Board breaking
  • Eating birthday cake and other refreshments
