I hereby certify that I am participating in a Montana Sheriffs & Peace Officers Association (hereafter referred to as “MSPOA”) sponsored or member training on behalf of my employing agency. I understand it is my obligation to disclose any existing injuries or physical limitations which may limit my performance or ability to participate in any simulated events related to critical incidents. The minimum fitness requirements will be no greater than those required for a SWAT basic training. I understand the basic minimum fitness levels and certify I am fit for any instruction or scenario without any restriction or accommodation. I understand it is my obligation to provide necessary individual equipment for training and/or team equipment as provided by my agency and MSPOA does not supply equipment. I am further disclosing that I will be participating on duty, in my capacity as an employee of the law enforcement agency listed on the registration form. I am waiving the MSPOA, as well as all instructors and their employers, from any claims or hereby releasing them from any and all claims, causes of action, damages associated with any tactical training or simulations.
