Please use this form to submit your proposed solutions for the STEM4Innovation and WA Innovation Hub Student Hospital Immersion Experience.

You'll still need to email a PDF copy of the full solution report to the organisers as outlined in other communications, but you'll also need to provide links to a publicly accessible version of the solution and the video presentation that the challenge has asked for. It should clearly reflect all the requirements of the Innovative Solution Report Rubric.

How you present the solution to the public is up to you - it might be a website, a shared document, an ebook, or other format. It is acceptable to embed the video element in your public facing component.
