Please fill out this form before March 27, 2020 to receive a screening appointment.


On Tuesday, April 21 and Thursday, April 23, Belding Area Schools will be conducting kindergarten registration and screening for children who will be at least five years old on or before September 1, 2020 (or five years old on or before December 1, 2020 by approved parent request) and wish to attend Belding Area Schools.  Please rank the time options listed below in your order of preference.  We will make every attempt to accommodate your first choice, but it is not guaranteed due to the number of students and parents we will be scheduling.  You must complete all answers before submitting the form and it will be time stamped as to when you submitted.  Information will be emailed to you regarding your appointment time.  When you arrive you will need to bring with you the following documents.


1.  Birth Certificate - Certified original birth certificate with the raised seal.  We cannot accept a copy of a birth certificate.
 
2.  Immunizations  - Please review with secretary

3.  Proof of Residency - Utility bill, lease agreement, property tax bill or purchase agreement.  We cannot accept a driver's license as proof of residency.

4.  Proof of Guardianship / Custody - If parents are divorced, a copy of court documentation showing responsibility.  If guardianship is applicable, documentation must be notarized.
