You must register online with the Fayette County Enrollment and Records Center prior to completing this form. If you have not yet registered, go to this website: www.fcboe.org/Page/237

After enrolling with the Fayette County Enrollment and Records Center, please complete this form to secure an appointment with a counselor to develop a McIntosh schedule for the 2023-2024 school year. New student registration appointments will be conducted in person at McIntosh High School.
