Absence Excuse Form

If you are reporting multiple children's absences please fill out one form per child.

Only three consecutive days can be excused with a parent note. After day 4 a doctor's note is required. All absences must be excused in writing. After 3 unexcused absences you will receive an attendance plan/contract and referral to a parenting class. You can access your child's attendance on parent portal at any time. Please feel free to email me with any questions.
