This form is due at least TWO MONTHS prior to your planned departure date and you must have approval before making further plans or travel arrangements.

Please fill out this form if you are the supervisor of a Phillips Academy student group, club, class, or team requesting support for short-term domestic overnight travel that requires at least one night away. Eligible programs include:

* a place-based learning opportunity off campus;
* a conference, event, competition, tournament, athletic training, or any other activity;
* an application for Phillips Academy- or Abbot Academy Association Fund (AAAF)-sponsored travel.

By submitting this form, you are committing to supervise/chaperone this program/event as the Program Director.

This form will go to Mark Cutler, acting as student travel coordinator, who will review the information with the Dean of Studies and the Dean of Students, in addition to the Travel Funding Oversight Committee (TFOC), and let you know if you and your students are approved to begin planning for travel.

Even after the travel plan is approved, please note that final clearance for each student will depend on the following:

* Adult/student ratios: Policy is 1:10 for domestic trips.
* Parent/guardian approval: Parents/guardians must sign waivers approving their children's/wards' participation.
* Student clearance: All participating students must be cleared by the Dean of Students and the Dean of Studies, in addition to members of the Sykes Wellness Center. Students are not eligible for travel if they are on active probation.
* Please note that students are permitted to miss up to five days of school for approved travel.
