Medical Needs While In-Person Testing
Please complete and submit this Google Form if your child has medical needs that will require staff attention while they are testing in-person at Carver Center ( 2671 Carver Road  Gambrills, MD 21054) on their assigned date of testing.  Our staff will work to ensure that your students is properly tended to.  We will have a Nurse on staff during each day of testing.  

**If you have multiple children enrolled at the Virtual Academy with medical needs that will need to be tended to during in-person testing, please complete a form for each child separately.**  

If your child requires medication to be administered on the day of in-person testing, please complete the medication form available by copying and pasting the link below.  Then, bring the completed medication at school form along with the medication on the day of testing.
