ELC - Fall Session 2023 Tutoring Student Form
THIS FORM IS FOR TUTEES / STUDENTS WHO WILL BE RECEIVING TUTORING: 
Evergreen Learning Chicago aims to connect student grades 4-8 with free tutoring through an enrichment program and a selective enrollment program (grades 6-8). By registering through this form, you're signing up / signing up your student to be connected with one of our tutors in order to receive support through the assigned semester. If you're signing up more than one student, please fill out this form once for each student. 

At the end of each semester, we ask that you refill out the form, and if you'd like to continue receiving tutoring, we ask that you indicate those needs and the tutors name as well. Please note that if the deadline for tutoring has passed, you can email us at evergreenlearningchicago@gmail.com, but being assigned a match will not be guaranteed. 
Your assigned tutor and you / your student will be sent a quick feedback survey after your first session. If there are any complications from the first session or later on, you can email us and we will do our best to re-match you with another tutor. 

As of now, tutoring is completely virtual. However, we may be hosting in person events and partnership tutoring sessions that will be in person this fall, especially for our Selective Enrollment Program.

a) The enrichment program is for homework help and targeted study plans for math/ELA/writing (4th-8th graders)
b) The selective enrollment program focusing on preparing students for the CPS selective enrollment test (ONLY 6th-8th graders; if your child is younger than this age group, they will automatically be paired with an enrichment tutor)

**Note: If your student necessitates both enrichment and Selective Enrollment aid, this will be two separate sessions, as Selective Enrollment Tutoring follows a strict curriculum without room for homework / enrichment aid.
