I understand how to access the Extracurricular Code of Conduct electronically and in print and agree to access the Code of Conduct and read it.  I understand that if I violate the rules, I can be disciplined.  Discipline may include suspension of the season, other behavior interventions, or other consequences.

If I have any questions about the content of the Extracurricular Code of Conduct, I will discuss my questions with the Assistant Principal for Student Life and Operations, Dr. Andrew Lambert.
