Please note in order to complete your application, a registration fee ($100) must accompany this form. $40 of this fee is refundable if there is no space for your child. If you are enrolling multiple children, an additional $40 will be added on per child to the previously mentioned fee.

This application should take no more than 15 minutes to complete. You will receive an email confirmation once your form has been submitted. Our Membership Chair will notify you if your child has received a spot in our program. If we are unable to place your child in a class, they will be added to our waitlist.  

**Please note that you will need the date of your child's last tetanus shot when filling out this form.**
