The purpose of this survey is to help us understand where teachers are in their use of formative assessment practices. Your individual responses to this survey will be confidential and will not be shared with RVUSD staff. Summary results will be shared with everyone participating in this project.

Formative Assessment Practices
There are four sections in this survey. Each section contains statements about classroom activities. Please select how frequently you and your students engage in each activity.

Section 1: Starts and Ends of Lessons

Select how frequently you do each of the following in your classroom.
