SEQUOIA BOOSTERS GRANT REQUEST FORM
The Sequoia High School Sport & Spirit Booster Club pursuant to its bylaws requires any school sports team requesting funds complete this Grant Application in its entirety.  The Grant Application must be made in person by the team coach and/or school Athletic Director at a Boosters Club meeting to address questions from the Boosters Club. Grant Applications should be submitted as early as possible in the academic year however, Grant Applications will be accepted year-round. The Executive Committee and eligible Boosters Members will vote on the Grant Application at the next regularly scheduled Boosters meeting. The Executive Committee reserves the right and has authority to award funds at any time.

The Boosters Club cannot pay individuals. A valid invoice or estimate from a company is required unless Boosters is reimbursing individuals for a pre-approved (budgeted) expense and a valid receipt has been turned in. Based on Boosters Club funds available and other factors approved Grants may be less than the amount requested.
