I have read and agree to abide by the rules and requirements set forth by USA Hockey in their publications, including but not limited to, Official Rules of Ice Hockey, Code of Conduct, Code of Ethics, and Zero Tolerance Policy.

I am providing my USA Hockey Registration number and agree to be added to the Team Roster of any and all teams for which I will appear on the bench prior to participating in any UWHL games or other UWHL events

USA Hockey does not require coaching certification for coaches who coach at the senior adult level (players who are age 18 or older). Registration on an official USA Hockey Roster, however, IS required. The UWHL encourages the professional development of all coaches as well as attendance at the classes provided by USA Hockey whenever possible.
