PASSPORT Puerto Rico Interest Form

Please complete this interest form for Stateside churches interested in attending PASSPORT Puerto Rico.  Early Registration will be open Tuesday September 26, 2023 8 am CST.

Submitting this form acknowledges that you have read and understand the following:
  • PASSPORT Puerto Rico is open to returning groups who have attended youth camp in 2023 or 2022.
  • Maximum capacity for one group is 12 Senior High students and 3 adults.
  • A Spring Familiarization (FAM) trip is required for each group leader. (Cost and date TBD)
  • Transportation in Puerto Rico is the responsibility of the group leader.
  • Air conditioned facilities in the worship space only. Housing is bunk rooms with hall baths. Dietary requests are not easily accommodated.
