In order to see the Nurse, Social Worker, School Counselor, or Career and College Counselor you will need to fill out this form.  We will do our best to see you as soon as possible by calling you down one by one.  

We will not be using a waiting area for any walk in sessions unless there is an emergency and you cannot remain in class for some reason.  Please understand that this is for everyone's health and safety.

We will either call you to the office when we can see you or contact you via email to discuss a good time to meet or answer any brief questions.  You may also always email us with questions that can be figured out remotely of course or message us via REMIND.
