KWMP Membership Form: January–December 2023
This form is to be filled out by anyone who is participating directly in the show (cast, crew, creative team) or an individual who would like to purchase a membership independently of the production. 

ONCE YOU COMPLETE THIS FORM, PLEASE SEND PAYMENT BY METHOD YOU CHOOSE AT THE BOTTOM OF THIS FORM.  *MEMBERSHIP IS NOT COMPLETE UNTIL BOTH THIS FORM AND PAYMENT ARE RECEIVED.*

ALL creative team, cast and crew members for productions are required to purchase a membership. Memberships are valid January 1–December 31 and entitle members who are of legal age to one vote at the AGM, as well as many other benefits!

Individual memberships cost $30
Individual memberships can be renewed by January 31st each year for only $25!
Family Membership (covers all household family members/maximum 2 adults over 18 years of age) cost $75   

* If you purchase or renew your membership for 2023 BEFORE JANUARY 31st, you will be entered into a draw for an incredible gift basket of goodies!!

Payment - $30 Individual, $25 for renewing members (if you were a member in 2022 you can renew for this rate before January 31st) or Family Membership $75. To be entered into the gift basket draw, payment MUST be received before January 31st at 6pm.
