I am seeking a makeup lesson. (Only a 24-HR notice from the time stamp of this form will be honored) *
The makeups are commonly held on a Sunday afternoon. Check all possible availabilities in the next 2 weeks (must be within the current Quarter). The makeup does not transfer to the next quarter unless you have a long-term medical condition**. If you are unavailable and would like to schedule further out within the current quarter, skip this part and email to re-request for dates.  This is a One-Time TEMPORARY scheduling. A 3-HOUR WINDOW IS MANDATORY. A ONE SPECIFIC TIME REQUEST IS NOT ACCEPTED.
