Talent Show 2023-2024 Permission Slip
Thank you for signing up to participate in the Linwood E Howe Talent show! This form needs to be filled out for each individual student that is participating. It is not one per family, not one per act, but one per individual student. Ideally, this form will be filled out before you audition, by February 6. But must be filled out for all participants by February 25.

Please download the attached document HERE and read it before filling out the form. It will answer most of your general questions, and let you know about procedures for this year's talent show.

It's going to be GREAT! We can't wait!

What is the student's name? *
School Permission Slip, required for ALL participants:

I, the grown up of the aforementioned student, grant permission for the student to participate in the Linwood Howe Talent Show, which includes but is not limited to: in person audition, rehearsal on stage, and the performance date. All school rules and regulations apply for this afterschool program. If permission is granted, please type your name below.

My child participates in a wednesday afterschool program on school property. *
Afterschool program permission slip; only required for students in an afterschool program on wednesdays.

I give permission for my student to potentially miss some or all of their afterschool program on March 13 for the talent show rehearsal. A rehearsal schedule will be created and dispersed to all relevant afterschool programs. Volunteers will help coordinate retrieving and returning students to their programs (ideally, students will not stay in the auditorium as it gets quite loud and chaotic). We will do our best to schedule cornucopia kids for after 2:45pm.

In person auditions on February 7 are first come, first served. We don't have the staff to retrieve students from afterschool programs, but we can return students to afterschool programs. Based on your audition preferences selected below, we will disperse the in-person audition list to all relevant afterschool programs for February 7 auditions.

Some students may also need to be dismissed early from their afterschool programs on friday in order to check in at the talent show performance on Friday March 15 at 5pm. If permission is granted, please agree below.

*Non-afterschool program kids can select N/A

What is the students' grade?
What are the student's pronouns?
Grown Up's email address. This should be an email you check with regularity, especially during the months of February and March. *
Any additional email addresses we should include with talent show communications?
What is the name of the act? *
Who else is in the act?
I understand the audition deadline for the 2023-24 Talent Show is February 11.

Any audition received February 12 or later will be automatically placed on the waitlist in order of receipt. Even if you submit the google form ahead of February 11. We must receive the actual audition by EOD February 11.
*
My act will audition via this method (only one audition needed per act) *
My act is available for the mandatory in person rehearsal on March 13, 2-6pm (not needed the whole time, you will be given a block of time to be present for) *
I have downloaded the talent show information packet. HERE (yes, it's the same one as above; just checking!) *
I have put all dates into my calendar. *
I agree to have my act participate in the show, or to decline participation with enough notice that a backup act may have the chance to join. *
I agree to turn in my tracked music, cut to the performance length, by the deadline of March 3 (via wetransfer is preferred, to linhowetalentshow@gmail.com; reach out if that option doesn't work). Acts without tracks should select yes. *
I agree to ask for help with music cuts (or anything else) early. I understand the talent show team will not be able to help with tracks during the week leading up to the show. *
The current performance length is 3 minutes. While we hope this won't happen, there COULD be a scenario where acts will be asked to trim their performances to 2.5 minutes. If this needs to be done, ample warning will be given in order to allow plenty of time for performers to adjust their act. *
Respect
1. I agree to give respect to get respect.
2. I agree to throw glitter, not shade.
3. I agree to arrive at talent calltime (or at a different time with permission) and to sign in to note my presence in the building.
4. I agree not to talk or move around the auditorium during an acts' performance.
5. I agree to stay until the end of the show to support all acts.
6. I understand the show is being run entirely by parent volunteers who are putting in an immense amount of work to make this happen for the kiddos.
7. I agree to have FUN!

I am interested in volunteering to help! (please and thank you!).
