Scholarship Transcript & Letter of Recommendation Form
Please Note:  This form is for requesting transcripts and letters of recommendation for SCHOLARSHIPS ONLY.

  • If you need these documents for a college application, you must submit a request through Naviance.

  • Transcripts include final grades, courses in progress, current grades where applicable, GPA and class rank.

  • Please submit a separate request for each scholarship you are applying for.

  • Transcripts can be picked up from the Counseling Office, Monday-Friday 8:00 a.m. -3:30 p.m.

  • You will receive an email for pick-up when the transcript and/or scholarship recommendation letter is ready.

PLEASE ALLOW 5 SCHOOL DAYS FOR TRANSCRIPT/RECOMMENDATION LETTERS TO BE PROCESSED
