Please fill out the following form to help us prepare for the upcoming school year.

High School Students:  All high school students will be issued a device for use during the school year.

Middle School Students:  In the event of a COVID-related school closure, all middle school students will be issued a device to use during that closure.  

Elementary Students:  In the event of a COVID-related school closure, the school district will provide a school-issued device to families who request one through this form.
