All art projects and events wishing to appear in the What When Where guide, and those seeking official placement on the event map must be registered using this form. Please note: This form is for both art and Events registration. If filling out for just art, put N/A in events fields. Vice versa if for events.  

All artwork and artists must abide by Element 11 policies. This includes but is not limited to the Mutant Vehicle Policy, Sound Policy, Laser Policy, and Fire Policy Please read and understand all policies prior to completing this form.
