All applications must be accompanied by either receipts or screen shots of items you will be purchasing with the money.

Educational Beginning of Year Grants can be used for purchases directly related to students' education. In order to reimburse for purchases, the PTA will need a copy of receipts and/or screenshot of amazon or other website purchases to show the money is being used for in-school, student-related activities, per NCPTA & IRS guidelines.

You may submit physical / printed copies of receipts to PTA Treasurer Jennifer Foster by leaving them in the PTA mailbox in the mailroom with this form attached: PTA Reimbursement Check Request Form or email the reimbursement form and receipts directly to SalemFinancials@gmail.com

I understand that my reimbursement is not automatic and is subject to WCPSS and NCPTA guidelines and review by the SMS PTA leadership team. I understand that they may have further questions. I agree to keep any receipts that I have related to the expenses claimed on this form in order to produce on state or federal tax documents if needed."
