Team Sponsorship Form

An annual team sponsorship is $1,000. You would be sponsoring a Fall Volleyball team, a Soccer team, a Basketball team, a Spring Volleyball team and a Little League team.

Individual team sponsorship is $250.00 per team sponsored.

If you don’t want to pay online, you can make a check payable to Fair Oak Youth Center and mail the check to Fair Oak Youth Center, PO Box 212, Fair Play, SC 29643. 

Your business name will be printed on the back of the team’s jerseys. Your business name will be on a banner in our gym for a year. Your business name will also be posted on our website for a year and we will make a Facebook post with all our season sponsors at the end of the season. 

All donations are tax deductible. Thank you for supporting the youth of our community through your sponsorship at Fair-Oak Youth Center.
