Photograph and Video Release Form
I hereby grant permission to the Big Walnut Education Foundation, its officers,
trustees, legal representatives. insurers, successors, assigns, and those
persons acting with their permission (collectively, the “BWEF”), to use photographs and recordings taken of my image, likeness, voice, appearance and/or performance (the “Product”) for publication, distribution and release in BWEF materials such as videos, display
boards, websites,
promotional brochures, newsletters, magazines, policies, manuals
and presentations, and to use the Product
and to offer the Product
for publication by other non-BWEF parties, without further advance notification.
I hereby waive any right to inspect
or approve the finished Product now or in the future, whether that use is known
to me or unknown. I hereby agree to release
and hold harmless the BWEF, including any firm publishing and/or distributing
the finished Product in whole or in part, from and against any claims, damages
or liability arising from or related to the use of the Product, including but
not limited to any misuse, distortion, blurring, alteration, optical illusion,
or in the taking, processing, reduction or production of the finished Product,
its publication or distribution.
I hereby warrant that I am of full age and have right, power and authority to execute this release on my own behalf. I have read this release before signing below, and I fully understand the contents,
meaning and impact
of this release. This release shall be binding upon me and my heirs, legal representatives,
successors and assigns.
